If you live in a facility (aging in place, ALF, ILF, SNF, or 55+ community) and want us to come to you:

• Please contact us with the name of your facility/community. 
• What will you get out of a Home Visit Consultation?
• We will come to your home and work with you individually
• We will advise you on how to make your home as safe as possible
• We will assist in incorporating what you learned in the group classes into your home environment
• Hands-on practice simulating daily activities to identify any problems or potential concerns.
